After having accessed documents held by the Office of the IGEM, should you find that your personal information is inaccurate, incomplete, out-of-date or misleading, you may make application for correction or amendment of any part of the information.
You can only amend those parts of a government document that contain your personal information.
Any such application must:
- have seen the document or have a copy of it
- provide enough information to identify the document
- provide an address (post or email)
- specify the information you want amended
- explain how the information is inaccurate, incomplete, out-of-date or misleading
- suggest how you want the information changed
- complete the Information Privacy Personal Information Amendment Application Form (available online at www.oic.qld.gov.au)
- provide evidence of your identity

If you are dissatisfied or "aggrieved" with the decision regarding access or amendment to documents you can apply to the Agency for an internal review of its decision, or alternately apply to the Information Commissioner for an external review of the decision.
